Booking
Our office requires a 48-hour notice for rescheduling or cancellation of your appointment. Failure to give a 48-hour notice or “No Show” your appointment, will result in a fee. A total of three no-shows/cancellations within 48 hours will result in termination as a client at JLG Medspa. All Filler and Sculptra will result in half the amount of your booked service(s). All other services will be assessed a $100 fee for any appointment cancelled/rescheduled less than 48 hours in advance. Any client who No Shows their initial appointment will not be permitted to re-schedule are unable to book any future appointments at our office with any of our providers.
If you are more than 10 minutes late for your service, we may not be able to accommodate you. In this case, the same cancellation fee will apply. We will do our very best to reschedule your service for another time that is convenient to you. In the event of a true, unavoidable emergency, we will work with you to accommodate rescheduling your appointment to a more convenient time.
We require a credit card on file to hold your appointment. Cancellation fees will be charged to your card on file.
Due to the nature of the services we provide, we reserve the right to reschedule your appointment if your child cannot sit quietly and independently. If your child cannot sit quietly and independently, it becomes a safety issue for providers, patient, and child. If your child is disruptive at your appointment, your appointment may be rescheduled at the provider's discretion.
If a second session or “touch up” is needed, it should occur 14 to 22 days after the initial treatment. In most cases, up to 4 additional units will be free of charge unless the nurse practitioner had advised additional units be administered at the initial visit and it was declined by the patient, at which time the patient may be responsible for each additional unit.
If there is a frequent history of second sessions or touch ups the patient will also be advised that the following routine treatment will require additional units. As always these are guidelines and each treatment and appointment will be decided by the nurse practitioner on a case-by-case basis.
Your form has been successfully submited to the JLG team. A member of our team will be in contact with you in a timely manner.
